MEMBERSHIP PRICING AND BENEFITS

  • $50 enrollment fee, $30 monthly membership.

    Enrollment fee includes a comprehensive review of your health history and current healthcare needs.

    Includes free telehealth visits for acute illnesses before 7pm daily. After 7pm cost is $50 per telehealth visit.

    Home visits incur $100 charge from 7am to 6pm. After 6pm $125 for an in- home visit.

    A great option if you need access for acute illnesses and do not have a primary provider or if you have a primary doctor that cannot see you quickly allowing you to avoid an urgent care or ER visit.

  • $50 enrollment fee and $50 a month.

    Enrollment includes an in-person comprehensive review of your health history and healthcare needs and initial set of labs are included.

    Includes a yearly physical. Lab testing for cholesterol screening, kidney and liver function, blood cell counts, vitamin D, thyroid function.

    $50 Phlebotomy fee if drawn in home.

    Telehealth on Saturday and Sunday is $40 per telehealth visit.

    Home visits

    $75 Monday through Friday.

    $125 after 6pm and on weekends.

    Weight loss included (medication is additional cost).

    Weight loss may require more frequent monitoring of labs which will be an additional cost that is not included in the monthly fee.

    Quarterly home visits included for medication review/management, nutritional education, preventative check up for chronic disease management (hypertension, diabetes, asthma, hyperlipidemia, etc).

    *Telehealth is included at no additional charge 7 days a week.

  • Without a membership plan.

    Sports physicals $25

    Telehealth appointments are $75 from 7am until 7pm, $125 after 7pm. These visits are performed via phone or video.

  • Labs can be drawn in home, at Labcorp, or your preferred laboratory. If you have health insurance you can utilize it to use it to cover the cost. Labs are offered at a significant discount if you do not have health insurance. Annual screening labs costing an average of $75-$125. This includes CBC (blood counts), CMP (electrolytes, kidney, liver function), thyroid function, cholesterol panel, vitamin D.

    The first set of labs is included with the Gold Membership enrollment fee.

    $50 phlebotomy fee is additional for in home labs.

Small business and family discounts available.

Membership based, no health insurance needed.

Free telehealth from 7a-7p with all membership levels.

In home or virtual visits available on the same day.

Weight loss therapy with semiglutide with counseling and support to reach your weight loss goals.

Laceration repair, abscess drainage, IV fluids, or medications administered during a home visit for an acute illness will have additional charge to cover the cost of supplies not to exceed $50.

SAVING YOU TIME AND THE EXPENSE OF AN ER OR URGENT CARE VISIT. WE SEE PATIENTS OF ALL AGES. WE DO NOT PROVIDE ROUTINE CHILDHOOD IMMUNIZATIONS.

  • Founder

    I am a married mother of three girls who are all grown and who have blessed me with beautiful granddaughters. I grew up in Saint Augustine from the age of 10, after my family moved from New York. I have been in Palm Coast since 2018. And I am looking forward to further serving this community.

I am the provider and owner of Coastal Concierge Healthcare. I started my career in 2006 as an RN working in nephrology, oncology, and then the emergency department. In 2012, I graduated from the University of South Alabama with a Masters in Science in Nursing specializing in family practice. Over the last 12 years as a board certified family nurse practitioner I have worked in internal medicine, as a hospitalist, and for the last 9 years in the emergency department.

I have felt a strong pull back to primary care after seeing so many patients in the emergency department that either lack access to primary care due to a lack of benefits or have limited access to their primary for sick visits which then prompts a visit to the emergency department or an urgent care center. I want to change that for my patients. I want to offer care that I would want to receive, personalized, attentive and comprehensive. I love establishing a personal relationship with my patients. Getting to know someone personally allows to better treat the person as a whole not just the disease or illness. I want to be able to promote whole body health and when the need arises to be there for you in times of illness. I look forward to meeting you and helping you meet your healthcare goals.
